Texas's Treatment Deserts: 132 Counties With Nothing

No state comes close to Texas in the scale of its treatment deserts. The 132 verified counties hold 1,786,729 people, more than the combined desert populations of Georgia, Virginia, South Dakota, and Missouri from our other state investigations put together. And these are not only empty ranchland counties: Liberty County outside Houston has 91,628 residents and no facility, Coryell County has 83,093, Henderson County 82,150. At the other extreme sits the scale problem money alone cannot fix: from Terrell County on the Rio Grande, the nearest listed facility is at least 115 straight-line miles away, in Odessa. In 15 counties, the nearest facility is not in Texas at all: it is in New Mexico, Oklahoma, Louisiana, or Arkansas.

Then we went one step further than the national report. On July 28, 2026 we asked the live locator, for every one of the 132 verified counties, which facility is closest to the county's 2020 Census population center, and how far away it sits. Those distances are straight-line miles, so every number in the table below understates the actual drive.

Check any county yourself: search its county seat on findtreatment.gov and note the location of each result. A radius search will usually return facilities, because it reaches into neighboring counties and states; the finding here is that none of them sit within the named county itself.

Texas's deserts follow three patterns, and each would need a different fix.

The West. West of a line from the Panhandle to the Pecos, the deserts are about raw distance. Nearly all of the 33 counties that are at least 50 miles from help sit in this half of the state, and all five of the counties at least 75 miles out do: Terrell (at least 115 miles), Brewster (92), Presidio (90), King (87), and Cottle (81). The entire Big Bend region, Brewster, Presidio, and Jeff Davis counties, an area about the size of Maryland, shares a single nearest provider: Permian Basin Community in Van Horn. And one outpatient center in San Angelo, the Alcohol and Drug Awareness Center, is the nearest facility for nine desert counties at once. If it ever closed, the nearest option for those nine would move even farther.

The border. The south Texas deserts are the populous ones. Starr County on the Rio Grande has 65,920 residents and its nearest facility is at least 42 miles away in Pharr; no other county on this list combines a population that size with a distance that long. Val Verde County, whose county seat Del Rio is a city of about 35,000, is at least 52 miles from help in Eagle Pass. Around the edges of the Rio Grande Valley, Brooks, Duval, Jim Hogg, and Kenedy counties are each 45 to 58 miles from the nearest facility. These are majority-Hispanic border counties where the overdose crisis meets the longest drives in the eastern half of the state.

The exurban ring. The third Texas hides in plain sight beside the big metros. Liberty County (91,628 people) and San Jacinto County sit on Houston's edge; Henderson County (82,150) is an hour from Dallas; Walker County (76,400) is home to Huntsville; Coryell County (83,093) sits beside Killeen. The distances are shorter here, but every one of these counties sends its patients into some other county's system while contributing zero capacity of its own. Along the eastern state line the pattern turns stranger still: from Newton and Sabine counties the nearest facility is in Louisiana, from Cass County it is in Texarkana, Arkansas, and from Red River County it is in Oklahoma.

Texas as a whole does not look underserved in state-level statistics: it has hundreds of facilities, concentrated in the big metros. That is exactly why the county-level view matters. Public behavioral health in Texas runs through regional authorities and nonprofit networks with multi-county catchments, organizations like PermiaCare in Odessa, Central Plains Center in Plainview, and Starcare in Lubbock, each of which appears in our table as the nearest provider for three to seven desert counties at once. The system is real, but for 1,786,729 Texans it exists only in the next county, or the next one after that.

Distance is not an inconvenience in treatment; it is a clinical variable. An outpatient program that expects attendance several times a week is arithmetic: at the median desert county's 36 straight-line miles, that is hours of driving per session, with no public transit, and for the 33 counties past the 50-mile mark it stops being a realistic plan at all. For the 15 counties whose nearest facility sits across a state line, a Texas Medicaid card may not open the nearest door.
