In these counties, a family confronting an overdose, a relapse, or a first honest cry for help has no local option to drive to. Whatever happens next happens somewhere else: another county, another town, often another hour or more away, assuming transportation, time off work, and childcare allow the trip at all. Research on treatment engagement is unambiguous that distance kills follow-through; every added mile between a person and a program lowers the odds they ever walk through the door.

How We Verified Every County
A claim like "this county has no treatment facility" is easy to make and easy to get wrong. So before publishing a single county name, we re-checked all 1071 candidate counties, one by one, against the live findtreatment.gov locator, the same tool anyone reading this can use. A county appears below only if the live federal locator, searched on 2026-07-12 with a radius covering the entire county, returned no facility located within it.
The starting list came from the SAMHSA facility dataset: 1,071 of the nation's 3,221 counties (33.3%) had no facility in the dataset. Live verification then removed 431 of them, in most cases because the locator now lists a facility the dataset snapshot did not include, or because a facility's ZIP code straddles the county line and we could not rule out that it sits inside. We chose to drop every such borderline county rather than argue about it. What remains is the strictest defensible list.
Check any county yourself: search its county seat on findtreatment.gov and note the county of each result. A radius search will usually return facilities, because it reaches into neighboring counties; the finding here is that none of them sit within the named county itself.

Why This Matters
County lines are not just administrative trivia in addiction treatment. Public funding streams, drug courts, probation requirements, and Medicaid managed-care networks are all organized at the county level, and a person's realistic treatment options are overwhelmingly shaped by what exists where they live. The overdose crisis has hit rural counties hardest precisely where the treatment map is emptiest.
This gap is also invisible in most national statistics, which report facilities per state or per capita. A state can look adequately served on average while a third of its counties have nothing. The county-level view is the one that matches how families actually experience the system, and it is the view this report exists to provide.

How we counted
Methodology
Methodology, sources, and limitations
Definition. A treatment desert here means a county containing no SAMHSA- listed substance use treatment facility within its boundaries. The claim is strictly boundary-based: a search by distance on findtreatment.gov may return facilities in neighboring counties, which does not contradict it.
Step 1, dataset. We started from the federal SAMHSA Behavioral Health Treatment Services Locator dataset (12,072 facilities, aligned with the 2024 N-SUMHSS survey cycle). Each facility was assigned to a county through its ZIP code using the U.S. Census Bureau's 2020 ZCTA-to-county relationship file, taking the county with the largest land-area overlap. 99.5% of facilities matched a county. This produced 1,071 candidate zero-facility counties out of 3,221 counties in 50 states, DC, and Puerto Rico.
Step 2, live verification. Every candidate county was then re-checked against the live findtreatment.gov locator on 2026-07-12: a substance use search centered on the county's 2020 Census population centroid, with a radius sized to cover the entire county (a minimum of 15 miles, larger for larger counties). If any returned facility had a ZIP code with any territorial overlap with the county, or an unplaceable ZIP code inside the county's radius, the county was excluded. This removed 431 counties and is deliberately conservative: a borderline case is dropped, not defended.
Limitations. Individual buprenorphine prescribers, private-practice clinicians, and telehealth-only providers are not SAMHSA-listed facilities and are outside both the dataset and the locator; some desert-county residents may reach care through those channels. ZIP-based county assignment is imperfect where ZIP codes cross county lines, which is exactly why the live verification step drops any such ambiguous county. Facility listings change continuously; the verification date above is part of the claim. County populations are 2020 Census counts.
