South Dakota's treatment desert is not a fringe of the state; it is most of the map. More than half of all counties have no substance use treatment facility inside their borders, and in the median desert county the nearest facility is at least 36 straight-line miles from the population center, before the road adds its own miles. Research on treatment engagement is unambiguous: distance kills follow-through, and every added mile between a person and a program lowers the odds they ever walk through the door.
Trust, but check
How We Verified Every County
A claim like "this county has no treatment facility" is easy to make and easy to get wrong. The federal SAMHSA dataset flagged 49 South Dakota counties as having no listed facility. Before publishing a single name, we re-checked all 49 against the live findtreatment.gov locator, the same tool anyone reading this can use, and dropped 11 of them because live verification could not rule out an in-county facility. Only the 38 counties that survived appear below.
Then we went one step further than the national report. On July 23, 2026 we asked the live locator, for every one of the 38 verified counties, which facility is closest to the county's 2020 Census population center, and how far away it sits. Those distances are straight-line miles, so every number in the table below understates the actual drive.
Check any county yourself: search its county seat on findtreatment.gov and note the county of each result. A radius search will usually return facilities, because it reaches into neighboring counties; the finding here is that none of them sit within the named county itself.
A hub city, the reservations, and the borders
The Three South Dakotas
South Dakota's deserts follow three distinct patterns, and each would need a different fix.
The hub city with nothing. Brown County (pop. 38,301) holds Aberdeen, one of the largest cities in the state, and the nearest substance use facility the federal locator can find is at least 77 miles away, near Watertown. That is not a rural edge case; it is a regional hub whose answer to addiction is a seventy-mile drive. The emergency room in Aberdeen functions as the treatment system because nothing else is there.
The reservation counties. Todd County (pop. 9,319), which lies within the Rosebud Sioux Reservation, is at least 82 miles from the nearest listed facility, in Pierre. Ziebach County, part of the Cheyenne River Sioux Reservation, is 71 miles out. And Corson County, the South Dakota portion of the Standing Rock Reservation, depends on the Standing Rock Treatment Program across the state line in Fort Yates, North Dakota. These are the communities carrying some of the heaviest substance use burdens in the region, and they are among the farthest from help.
The border counties. For 10 of the 38 counties, the nearest facility is not in South Dakota at all. The Standing Rock Treatment Program in Fort Yates, North Dakota is the closest option for five different South Dakota counties. Union County leans on Sioux City, Iowa; Gregory and Douglas counties on Nebraska. A quarter of the state's desert counties depend on a facility operating in another state's treatment system, funding streams, and Medicaid network.
Meanwhile the state's own capacity concentrates in its southeastern corner: roughly a third of all SAMHSA-listed facilities in South Dakota sit in Sioux Falls alone. Turner, Union, and Clay counties, all verified deserts, ring that metro and compete for its waiting lists.

Distances are straight-line miles from the county's 2020 Census population center to the nearest facility listed by the live federal locator, and understate real driving distance. A nearest facility in a neighboring county does not contradict the finding: the county itself contains none.
Context
Why This Matters
County lines are not administrative trivia in addiction treatment. Public funding streams, drug courts, probation requirements, and Medicaid managed-care networks are all organized at the county level, and a person's realistic treatment options are overwhelmingly shaped by what exists where they live. For the ten border counties above, even the nearest facility sits in another state's system entirely.
This gap is invisible in most state-level statistics. On a per-capita basis South Dakota can look modestly served; the county-level view is the one that matches how families actually experience the system. 253,423 South Dakotans live in a county where the window in which someone finally says yes to treatment, a window that usually lasts hours, has to survive a drive measured in half-tanks of gas.

How we counted
Methodology
Methodology, sources, and limitations
Definition. A treatment desert here means a county containing no SAMHSA-listed substance use treatment facility within its boundaries. The claim is strictly boundary-based: a search by distance on findtreatment.gov may return facilities in neighboring counties, which does not contradict it.
Step 1: the dataset. We started from the federal SAMHSA Behavioral Health Treatment Services Locator dataset (12,072 facilities nationally, aligned with the 2024 N-SUMHSS survey cycle). Each facility was assigned to a county through its ZIP code using the U.S. Census Bureau's 2020 ZCTA-to-county relationship file, taking the county with the largest land-area overlap. This flagged 49 of South Dakota's 66 counties as zero-facility candidates.
Step 2: live verification. Every candidate county was re-checked against the live findtreatment.gov locator on July 12, 2026: a substance use search centered on the county's 2020 Census population centroid, with a radius sized to cover the entire county. If any returned facility had a ZIP code with any territorial overlap with the county, the county was excluded. This removed 11 South Dakota counties and is deliberately conservative: a borderline case is dropped, not defended.
Step 3: nearest-facility distances. On July 23, 2026, for each verified county, we queried the live locator outward from the county's 2020 population centroid (30, 60, then 120 mile radius) and computed the straight-line distance to every returned facility from its listed coordinates, keeping the closest. Straight-line miles understate driving distance, so all copy on this page says "at least".
Limitations. Individual buprenorphine prescribers, private-practice clinicians, and telehealth-only providers are not SAMHSA-listed facilities and are outside both the dataset and the locator; some desert-county residents may reach care through those channels. Facility listings change continuously; the verification dates above are part of the claim. County populations are 2020 Census counts.
